Hospice care is the place for nurses looking for the autonomy to spend time with their patients, manage their own care plans and lead an interdisciplinary team. Responsibilities

• Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
• Develop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions, including the patient and family in the planning process.
• Initiate appropriate preventive and rehabilitative nursing procedures and administer medications and treatments as prescribed by the physician in the physician's plan of care.
• Travel throughout the day and sometimes after-hours.

Requirements

• RN licensure for the state(s) you will work in.
• Clean driving record, and valid driver's license and auto insurance.
