About the position
Hampton Falls School District is actively seeking a Special Education Case Manager/Teacher to work with students by ensuring each student's IEP is being followed, accommodations and modifications are implemented, and specialized instruction and related services are delivered per the Individual Education Plan. In addition, the prospective candidate will help facilitate learning in inclusive settings, participate as a member of the student's IEP team to design and implement programming to ensure student growth.
Responsibilities
- Ensure each student's IEP is being followed.
- Implement accommodations and modifications as needed.
- Deliver specialized instruction and related services per the Individual Education Plan.
- Facilitate learning in inclusive settings.
- Participate as a member of the student's IEP team.
- Design and implement programming to ensure student growth.
- Case manage +/- 15 students.
- Use NHSESIS to generate IEPs.
- Provide training to classroom staff and support personnel.
- Communicate effectively with regular educators, parents/guardians, educational associates, and administration.
- Collect and organize academic and/or behavior data.
