Bilingual Case Manager CM

About the position

Conifer Park is seeking a full-time Bilingual (English/Spanish) Case Manager to join our clinical team in Glenville, NY. In this role, the employee assumes full charge of patient case management, delivering individual and group mental health and chemical dependency services including assessment, discharge planning, managed care, and relevant communication, formulation and implementation of treatment plans, and performing specified duties. Schedules: Mondays - Fridays 8:00am -5:00pm.

Responsibilities

• Assume full charge of patient case management.
• Deliver individual and group mental health and chemical dependency services.
• Conduct assessments and discharge planning.
• Manage care and relevant communication.
• Formulate and implement treatment plans.
• Perform specified duties as required.

Requirements

• High School Diploma or GED with a minimum of 1 year of Case Management experience or applicable internship and a CASAC-T certification for Case Manager I.
• High School Diploma or GED with 1 year of Case Management experience or applicable internship and possess a current CASAC Certification for Case Manager II.
• Bachelor’s Degree and current CASAC certification OR Master’s licensable degree and a minimum of 1 year relevant experience or applicable internship for Case Manager III.
• Master’s Licensed Degree with LMHC, LMSW or LCSW for Case Manager VI.
• Experience in Chemical dependency and/or mental health.
• Experience in Group counseling and Didactic Skills.
• Ability to demonstrate Bilingual skills.
