Responsibilities

  • Conduct evidence-based diagnostic evaluations (primarily via telehealth) for individuals presenting with signs of dementia or other neurocognitive disorders
  • Perform intake assessments for new patients entering Rippl’s care programs
  • Utilize cognitive, behavioral, and psychopharmacological assessment tools to support accurate diagnosis
  • Clearly communicate diagnostic findings to patients, caregivers, referring providers, and the internal Care Team
  • Develop and implement individualized treatment and care plans tailored to patient and caregiver needs
  • Provide intermittent dementia-specific clinical visits, including medication adjustments for cognitive or behavioral symptoms and consultation for escalation in dementia-related behaviors
  • Support reassessments as needed for patients experiencing significant change in condition
  • Monitor caregiver strain and incorporate caregiver support into the overall plan of care
  • Partner closely with Rippl Care Navigators, Social Workers, and Medical Directors
  • Coordinate care with patients' existing medical teams, including primary care and specialists
  • Ensure timely and compliant documentation of clinical encounters and care recommendations
  • Adhere to all legal and ethical standards of clinical practice

Requirements

  • Current, unrestricted license to practice as an ARNP in one of the following states: WA, TX, or FL (willingness to license in other states a plus)
  • Geriatric Medicine, Palliative Care or Chronic Care management experience preferred
  • Active Drug Enforcement Administration (DEA) license or ability to apply for such license prior to or upon hire if requested
  • Strong diagnostic and clinical assessment skills with experience managing patients with cognitive decline or dementia
  • Excellent communication and interpersonal skills, with the ability to establish rapport and build therapeutic relationships with patients and their families
  • Proficiency in electronic health record (EHR) systems and other healthcare technology tools (Athena preferred)
  • Commitment to professional growth and ongoing learning in the field of psychiatric-mental health nursing
