About the position

The RN Case Manager provides case management for assigned patient populations, utilizing clinical expertise, communication, and problem-solving skills to achieve optimal clinical and resource outcomes. The role promotes cost-effective care by minimizing fragmentation, maximizing coordination, and facilitating patient/family movement through the healthcare organization. Responsibilities include performing patient needs assessments, facilitating referrals, and providing linkages to health, wellness, and post-acute care resources. The RN Case Manager collaborates with interdisciplinary teams to progress care plans and discharge plans, ensuring compliance with federal and state regulations surrounding the discharge process. The position requires knowledge of growth and development appropriate to the age group served and the ability to incorporate this knowledge into the care plan.

Responsibilities

  • Collaborates with Utilization Review Nurse.
  • Facilitates an interdisciplinary approach to patient care.
  • Coordinates care and services within the case managed population.
  • Performs face-to-face assessments of patients/families to identify individualized needs.
  • Documents in the medical record accurately and in a timely manner.
  • Facilitates appropriate referrals for high-cost medications.
  • Ensures coordination of care during patient transfers.
  • Advocates for the patient and family throughout the care episode.
  • Participates in departmental and system performance improvement initiatives.
  • Coordinates referrals for DME, HH, and Hospice.
  • Utilizes predictive analytic software.
  • Communicates with assigned UR nurse and SW partner.
  • Reassesses patients and documents status of referrals.
  • Aids in the delivery of regulatory letters.
  • Integrates InterQual information during unit huddles.
  • Provides Medication Assistance to patients in need.
  • Initiates Medication Investigations.
  • Communicates post-acute care needs of inmates during transitions.
  • Maintains awareness of unit-based patient needs.
  • Provides hand-off communication of unit needs during transitions.

Requirements

  • Registered Nurse with a Bachelor's degree required.
  • 5 years of Case Management experience in a hospital setting may be considered in lieu of a Bachelor's Degree.
  • Three years of recent experience in a clinical healthcare setting with direct management of patient care.
  • Supervisory or leadership experience is preferred.
  • Current licensure in Virginia as a Registered Nurse.
  • Demonstrated knowledge and competency in communication skills, integrity, innovation, and teamwork.

  • Experience with community and system resources.
  • Knowledge of computer literacy and effective interpersonal relations.
  • Ability to resolve complaints/problems and a customer-focused philosophy.
