About the position

The position involves facilitation, coordination, and communication between patients, families, and referral sources to maximize the patient's functioning capacity in the community. The role requires independent judgment and discretion, making patient management decisions based on knowledge of the organization, policies, procedures, and professional ethics. The position is performed under minimal supervision.

Responsibilities

  • Assess hospice patients' psychosocial, financial, environmental, and community needs.
  • Provide direct and indirect social services support for patients and families in various service settings.
  • Deliver crisis intervention and supportive counseling for patients and families adjusting to death and terminal health conditions.
  • Complete all documentation in compliance with federal/state hospice regulations and policies.
  • Actively participate as a member of the interdisciplinary team, providing input to support patient/family care objectives.
  • Attend team meetings, contribute to care planning, and collaborate with colleagues.
  • Provide consultation and assistance to colleagues, administration, patients/families, and community partners.
  • Support level-of-care changes and discharge planning for patients and families.
  • Respond promptly to referrals from the interdisciplinary team and community for social services needs.
  • Accept assignments consistent with education, experience, and competence.
  • Promote teamwork, effectiveness, and efficiency through a cooperative and professional attitude.

Requirements

  • Master's Degree from an accredited School of Social Work.
  • Minimum 1 year of experience delivering social work services in a healthcare setting.
  • At least 3 years of experience in inpatient, outpatient, or home-based social work.
  • Demonstrated experience in palliative care settings, including independent and collaborative work within an interdisciplinary team.
  • Palliative assessment and management of physical, psychosocial, and spiritual needs during aggressive treatment and end-of-life care.

Nice-to-haves

  • Knowledge of research techniques, data collection, policy/program/curriculum development.
  • Experience with quality improvement methodology.
  • Post-degree experience in a hospital or other healthcare setting.
