Vitality NP Services is seeking a Nurse Practitioner to join their team in a skilled nursing facility in Yeadon, PA. This full-time position offers flexible hours with no weekend, evening, or on-call requirements, focusing on the assessment and care of subacute and long-term care patients.

Responsibilities

• Assessment of all recent admissions and acute issues of both subacute and long-term care patients
• Prevention of needless hospital transfers through early assessment and interventions
• Communication and collaboration with primary care and specialists

Requirements

• CPR Certification
• RN License
• Certified Nurse Practitioner
• DEA Certification
• Excellent communication skills
• Ability to work as an interdisciplinary team member
• Good critical thinking and assessment skills
• Ability to function in a fast-paced environment
• Excellent written and verbal communication skills
• Proficiency in computer skills
