About the position

The Case Manager PRN at Anderson Rehabilitation Institute plays a crucial role in the interdisciplinary team, focusing on the care and recovery of patients in an inpatient acute rehabilitation setting. This position involves collaborating with various healthcare professionals to ensure comprehensive patient care, facilitating communication with patients and their families, and maintaining accurate documentation to support the rehabilitation process.

Responsibilities

• Completes departmental orientation, initial and annual competencies.
• Consults with other departmental staff to collaborate in patient care delivery, identify barriers to care and discharge, and develop solutions.
• Completes documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
• Schedules family conferences and communicates with caregivers to keep them informed of patient progress and rehabilitation plans.
• Coordinates weekly patient care team conferences to develop, monitor, and refine treatment plans to achieve patient goals.
• Reviews the patient's assigned CMG and identifies any potential missed comorbid conditions, communicating findings to the HIM team.
• Performs other duties as assigned.

Requirements

• Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
• Certification in Case Management or Rehabilitation Nursing preferred (e.g., CCM, ARN, ACM, RN-BC).
• Minimum of 2 years social work or case management experience in an inpatient setting highly preferred; acute/rehabilitation hospital experience preferred.
• Effective oral and written communication skills in English; additional languages preferred.
• Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required.

Nice-to-haves

• Certification in Case Management or Rehabilitation Nursing (e.g., CCM, ARN, ACM, RN-BC) is preferred.
• Experience in acute/rehabilitation hospital settings is preferred.
