About the position

The Case Manager at Children's Home of Reading Youth & Family Services (CHOR YFS) is responsible for maintaining resident medical records and individual service plans, ensuring compliance with regulatory standards. This role involves coordinating healthcare appointments, facilitating in-home visits, and mentoring direct care staff, all while providing support to children and families in need of behavioral and mental health services.

Responsibilities

  • Maintain all client records and medications utilizing the Electronic Health Record system (Credible).
  • Ensure emergency medical and emergency contact information is up to date on the Electronic Health Record for all clients.
  • Review and approve all medical incident reports.
  • Schedule and track all routine, specialist, and follow-up appointments, ensuring compliance with medical regulations set forth by all licensing and regulatory bodies.
  • Secure private and/or medical assistance health insurance information, if not supplied at the time of intake.
  • Order, maintain, and monitor prescription medication, ensuring required doses are available for clients as prescribed.
  • Oversee administration of medication, and train necessary staff in mandated protocols.
  • Notify parents and/or legal guardians of all client medical appointments and changes in client medications.
  • Keep and maintain the ISP portion of the resident's record ensuring compliance with regulations set forth by all licensing bodies, referring counties, and Managed Care organizations.
  • Notify all involved parties of scheduled ISP meetings, 30 days in advance of the meeting.
  • Document notifications in EHR.
  • Complete behavior adjustment portion (goals and summary) of treatment plans, integrating goals identified and submitted by Primary CCWs.
  • Mentor direct care staff concerning creating and adjusting behavioral goals.
  • Update daily sheets, ensuring accurate behavioral goals are included.
  • Attend and help facilitate all ISP meetings.
  • Coordinate and attend meetings with counties and stakeholders.
  • Oversee the orientation and ongoing training of all direct care staff.
  • Obtain and maintain certification as a MAB (Management of Aggressive Behavior) trainer.
  • Coordinate Motivational Interviewing (MI) training for all residential staff.
  • Schedule training for coaches.
  • Provide CPR and 1st Aid training to all employees as required.
  • Maintain records of timelines associated with all employees in order to ensure compliance.
  • Obtain and maintain certification as a Medication Administration trainer.
  • Assist in medical appointment transportation, when necessary.
  • Periodically fill in for Child Care Workers and Supervisors, when necessary.
  • Perform other job duties as assigned.

Requirements

  • Bachelor's degree with a background in the creation and implementation of behavioral modification goals.
  • CPR Certification.
  • Experience in motivational interviewing.
