About the position

The Case Manager at Project Redirect Inc. is responsible for providing comprehensive, person-centered services to a caseload of approximately 20-25 clients. The role focuses on helping clients achieve fulfilling lives in the community through counseling, case management, and aftercare services. The Case Manager will collaborate with colleagues to meet client needs and ensure program success while adhering to the organization's core values and vision.

Responsibilities

  • Provide a full range of on-site, person-centered, strengths-based services to clients.
  • Conduct initial intake assessments and respond to critical needs appropriately.
  • Orient clients to the shelter program, including requirements and consequences.
  • Develop and provide clients with an Independent Living Plan (ILP) in collaboration with other service providers.
  • Assist non-working clients in obtaining Public Assistance.
  • Complete psychosocial assessments within the first 30 days of intake.
  • Track and monitor client progress and empower them in planning and goal setting.
  • Conduct weekly or bi-weekly meetings with clients as needed.
  • Refer clients to appropriate resources and encourage job development for employment opportunities.
  • Maintain client files, documenting conversations, warnings, and progress reports.
  • Report critical incidents to the Site Director or Director of Social Services.
  • Collaborate with Housing Specialists and Employment staff to assess client readiness for employment or housing.
  • Treat all clients, visitors, and employees with kindness, respect, and dignity.
  • Provide case management and counseling for one year beyond the client's departure.
  • Develop and maintain connections with other programs for inter-agency referrals.

Requirements

  • Baccalaureate degree in Social Work, Psychology, Sociology, or related field with 2+ years of direct social service experience.
  • Associate degree in Human Service or related field with 4+ years of direct social service experience.
  • High School Diploma/General Equivalent Diploma with 8+ years of direct social service experience.
  • Strong interpersonal skills with the ability to be compassionate and firm while maintaining confidentiality.
  • Knowledge of community resources and ability to solve problems, make decisions, and resolve conflicts.
  • Fluency in English and Spanish is a plus.
  • Ability to remain calm in crisis situations and work collaboratively with diverse groups.
  • Proven ability to handle multiple tasks effectively under pressure.
  • Strong organizational skills, detail-oriented, and efficient.
