About the position

The Human Services Coordinator I (HSC I) position at the Department of Developmental Services (DDS) serves as the primary contact for individuals and families, facilitating the development of Individual Support Plans and ensuring the delivery of appropriate services. The role emphasizes building collaborative partnerships with various stakeholders and monitoring the well-being of individuals served by the department.

Responsibilities

  • Coordinate and facilitate the development of Individual Support Plans according to DDS regulations and individual needs.
  • Provide coordination and support to individuals enrolled in traditional and self-directed services.
  • Identify issues and concerns, documenting steps taken to resolve them.
  • Monitor the safety and well-being of assigned individuals through regular contact and site visits.
  • Provide information and education regarding DDS services and community resources to individuals, families, and guardians.
  • Assist individuals with applications for state plan services and federal entitlement programs for people with disabilities.
  • Ensure individuals receive appropriate services and arrange referrals as needed.
  • Monitor situations where individuals are at risk and arrange for crisis intervention when necessary.
  • Maintain accurate records in Virtual Gateway and Meditech while ensuring confidentiality.
  • Collaborate closely with Area Office staff and provide information to the management team.

Requirements

  • At least three years of full-time or equivalent part-time professional experience in human services work or social work.
  • A Bachelor's degree or higher in a related field may substitute for two years of required experience.
  • A valid motor vehicle driver's license may be required based on assignment.

Nice-to-haves

  • Experience working with individuals with intellectual and developmental disabilities.
  • Knowledge of state and federal entitlement programs for people with disabilities.
