The Medical Social Worker will provide medical social services under the direction of a physician, based on the patient’s psychosocial assessment and the patient and family’s needs. The social worker will participate in the coordination of care with the Interdisciplinary Group/Team (IDG/T) and participate in on-call rotation per the Hospice needs.
Responsibilities
- Provide medical social services under the direction of a physician.
- Conduct psychosocial assessments based on patient and family needs.
- Coordinate care with the Interdisciplinary Group/Team (IDG/T).
- Participate in on-call rotation as needed.
Requirements
- MSW degree from an accredited school of social work, or BSW degree from an accredited institution, or a related field degree with supervision by an MSW.
- One year of social work experience in a healthcare setting.
- Licensure as required by the state where practicing.
- Current driver's license.
- One year of experience in palliative care or hospice preferred.
- Working knowledge of community resources.
- Good interpersonal skills.
- Ability to work with computers.
- Reliable transportation and valid auto liability insurance.
