Medical Social Worker Full-Time (BSW/MSW)

The Medical Social Worker will provide medical social services under the direction of a physician, based on the patient’s psychosocial assessment and the patient and family’s needs. The social worker will participate in the coordination of care with the Interdisciplinary Group/Team (IDG/T) and participate in on-call rotation per the Hospice needs.

Responsibilities

• Provide medical social services under the direction of a physician.
• Conduct psychosocial assessments based on patient and family needs.
• Coordinate care with the Interdisciplinary Group/Team (IDG/T).
• Participate in on-call rotation as needed.

Requirements

• MSW degree from an accredited school of social work, or BSW degree from an accredited institution, or a related field degree with supervision by an MSW.
• One year of social work experience in a healthcare setting.
• Licensure as required by the state where practicing.
• Current driver's license.
• One year of experience in palliative care or hospice preferred.
• Working knowledge of community resources.
• Good interpersonal skills.
• Ability to work with computers.
• Reliable transportation and valid auto liability insurance.
