About the position

Optum is seeking a Nurse Practitioner to join the Home-based Medical Care team in Bartow County, GA. This role focuses on delivering home-based medical care to complex, chronic patients, particularly older adults, enhancing their quality of life by providing comprehensive medical services in their homes. The Nurse Practitioner will collaborate with an interdisciplinary team to develop and implement care plans, ensuring a seamless health journey for patients.

Responsibilities

  • Practice in accordance with state laws and regulations governing advanced practice nursing.
  • Establish and maintain effective relationships with patients, caregivers, and interdisciplinary team members.
  • Perform initial comprehensive assessments on newly enrolled patients and provide ongoing care.
  • Develop and update patient management care plans as needed.
  • Meet with patients and their representatives to review care plans, involving the PCP when applicable.
  • Complete follow-up and post-discharge assessments according to standard operating procedures.
  • Prescribe appropriate diagnostics and interventions to avoid unnecessary acute admissions.
  • Perform basic nursing procedures such as IV placement, blood draws, and wound care.
  • Consult with hospital and emergency clinical teams following patient transfers.
  • Educate patients and their representatives on disease processes and wellness management.
  • Provide counsel and support to staff as appropriate.
  • Participate in ongoing meetings related to patient care and clinical excellence.
  • Implement HEDIS measure campaigns and quality initiatives to improve care management.
  • Be on-call for caseload during business hours and participate in after-hours on-call program.

Requirements

  • Completion of Nurse Practitioner accredited program.
  • Current state RN license.
  • Current NP license.
  • Access to reliable transportation.

Nice-to-haves

  • 2+ years clinical experience in primary care, home-based medical care, geriatrics, palliative care, or emergency/urgent care.
  • Knowledge of long-term care and community resources.
  • Familiarity with CPT, ICD-10, and HCPC codes.
  • Understanding of clinical standards of care and NCQA requirements.
  • Awareness of CMS guidelines and Medicaid/Medicare contracts.
