About the position

The Nurse Practitioner will function within the Primary Care Service of the VA Northeast Ohio Healthcare System, providing comprehensive primary care services to a defined panel of patients. This role involves leading a Patient Aligned Care Team (PACT) and requires a high level of medical knowledge, excellent interpersonal and leadership skills, and the ability to provide compassionate care to veterans. The Nurse Practitioner will be responsible for clinical examinations, diagnosis, treatment of acute and chronic diseases, and coordination of care, while also ensuring adherence to VA standards and performance measures.

Responsibilities

  • Provide comprehensive primary care services to a defined panel of patients.
  • Conduct history taking and physical examinations.
  • Order laboratories and diagnostic studies.
  • Diagnose and formulate treatment plans for acute and chronic conditions.
  • Lead a Patient Aligned Care Team (PACT) consisting of a physician, RN, LPN, and clerical associate.
  • Monitor PACT teamlet performance to assess adherence to VA standards and quality of care.
  • Document patient care appropriately in clinical records.
  • Refer patients to specialists when necessary.
  • Assist in patient care or administrative duties at the main hospital or other sites as needed.
  • Resolve patient concerns and inquiries in collaboration with patient advocates and administration.

Requirements

  • United States Citizenship.
  • Proficiency in basic written and spoken English.
  • Graduate of an accredited school of professional nursing.
  • Current, full, active, and unrestricted registration as a graduate professional nurse.
  • Licensure as a nurse practitioner in a State.
  • Master's degree from an accredited program in nursing or related field.
  • Full and current certification as a nurse practitioner from a recognized certifying body.
