Responsibilities

• Manage clinical care of the population at assigned SNFs.
• Round daily and prescribe medications and treatments for residents.
• Provide comprehensive primary care services to patients in SNFs.
• Timely documentation of all patient care encounters.
• Available for phone/secure messaging consultations with facility nursing staff.
• Complete documentation in Theoria's proprietary EHR system, ChartEasy™.
• Utilize facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech.
• Ensure on-time attendance for scheduled shifts.

Requirements

• Active Nurse Practitioner license in the state of practice.
• Experience in providing primary care services, preferably in a post-acute setting.
• Strong communication skills for patient and staff interactions.
• Ability to manage time effectively and maintain punctuality.

Nice-to-haves

• Experience with electronic health record systems such as ChartEasy™, PointClickCare, MatrixCare, or American HealthTech.
• Familiarity with skilled nursing facility operations.
