About the position

The Registered Nurse (RN) Care Manager for Home Health at PruittHealth @ Home is responsible for providing and coordinating skilled nursing care under physician supervision. This role emphasizes the delivery of competent nursing interventions to home health patients, ensuring continuity of care through collaboration with an interdisciplinary team. Responsibilities

• Provides competent nursing care & interventions to home health patients according to state and industry standards.
• Supervises care provided by Home Health Aides and Licensed Practical Nurses assigned to the care team.
• Collaborates with the interdisciplinary team and attending physician to develop and manage the patient care plan.
• Conducts field visits to evaluate and initiate care, and monitor clinical outcomes.
• Utilizes accepted company teaching materials in patient/family education.
• Assesses, monitors, documents, and reports progress of patient's health and condition.
• Completes documentation timely and accurately according to industry standards.
• Honors patients' rights and ensures adherence to physician-approved care plans.
• Complies with corporate compliance program and meets agency productivity standards.
• Participates in center/agency surveys, mandatory in-services, and staff meetings.
• Follows established safety regulations and procedures, reporting job-related hazards as necessary.
• Maintains current license, certification, and clinical records to meet regulations and guidelines.
• Manages time effectively to meet outcomes and strategic goals.

Requirements

• Nursing degree from an accredited college or university, or graduate of an accredited/approved RN program.
• Minimum of one (1) year of RN experience.
• Valid driver's license and reliable transportation.
• Liability insurance to complete home visits.
• Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice.
• Current CPR certification.

Nice-to-haves

• Bachelor of Science in Nursing (BSN) preferred.
• Acute care experience with adult or aging populations.
• Home health and/or skilled nursing experience.
